The transformation of health services in rural areas is one of the main challenges in efforts to equalize health access in Indonesia. Limited medical personnel, difficult geographical distances, and lack of health infrastructure often hinder rural communities from obtaining quality services. Telemedicine is present as an innovative solution by utilizing communication technology to bring together patients and health workers without space and time limitations. Furthermore, the integration of artificial intelligence (AI) in telemedicine systems is able to improve the effectiveness of diagnosis, personalization of services, and the efficiency of health data management. This qualitative research aims to explore trends in the use of AI-based telemedicine in expanding access to healthcare in rural areas, while identifying emerging opportunities and challenges. The results of the study show that the application of AI-based telemedicine can speed up the clinical decision-making process, improve service quality, and expand the reach of services to remote areas. However, there are obstacles in the form of limited public digital literacy, the readiness of network infrastructure, and regulatory and ethical aspects of the use of health data. Thus, the integration of telemedicine and AI is not just a technological innovation, but a health system transformation strategy that has the potential to strengthen the equitable distribution of services and improve the quality of life of rural communities.
